My boyfriend once called me by his ex's name and I was surprised but honestly didn't care that much, he apologised loads but I was fine.

Give him the benefit of the doubt I would say I sometimes go through spells of name mix-ups with my friends without any real connection or reason for swapping their names. I know it's a bit different with a previous partner, but you might be reading too much in to things - just because he mixed up some names, doesn't mean he's madly in love with his ex or that you are her replacement.
